Duke Health has partnered with Cadence to bring you Duke Health Connected, a service that helps your care team manage your health between office visits with at-home health monitoring.

What is Duke Health Connected?

Duke Health Connected helps you stay connected with your Duke Health provider. Using simple devices, you can measure important health information at home. Your care team checks these measurements to:

Catch health changes early
Reduce trips to the hospital
Better manage ongoing health conditions
Help you feel better day-to-day

How it Works

Getting Started: Your Duke Health provider will recommend the program for you if they think it could have a positive impact on your health. You will then be offered enrollment to the program, which is powered by Cadence. If you choose to enroll, you'll receive an easy-to-use monitoring device and speak directly with our care team.
Daily Checks: Take quick measurements from the comfort of your home as directed by your care team. For patients with heart conditions or high blood pressure that would be a blood pressure monitor or weight scale. For patients with high blood sugar that would be a glucose meter to check your blood sugar levels.
Automatic Updates: Your measurements are automatically and securely sent to your care team without you having to do anything extra.
Personal Support: The care team will check in with you regularly during telephone visits. If any of your readings indicate a potential concern, the care team will contact you right away to help.

Common Questions

Does insurance cover this?
Yes. The program is generally covered under most insurances, after deductibles are met. Your care team will review your health plan coverage during the enrollment call. 
How do I sign up?
If your doctor recommends this program, you can call or text the Cadence Care Team at 919-351-8376.
Is my health information safe? Yes. All of your health information is protected and secure under HIPAA and Duke Health privacy policies.
