Does your child need a sports physical? If they plan to work out with their high school team this fall, participate in a fall sport, or try out for a sport any time in the upcoming school year, now is the time to make sure your forms are up to date.
School Sports Physicals for $10
Important Information
- Duke Health will offer school sports physicals to rising seventh- through twelfth-grade student athletes.
- Physicals are $10 (cash only, exact change).
- Athletes MUST bring either the NCHSAA or NCISAA Pre-participation Physical Evaluation Form with the History Form (pages 1 and 2) completed and signed by a parent or legal guardian. You can also obtain forms from your school’s athletic trainer or athletic department.
- A minor may be seen without a parent present with a completed permission form.
- Parking is free.
