Duke Health is participating in ACCESS, a new Medicare program that provides eligible patients with extra support for managing chronic conditions—at no cost to you.

You may hear from ACCESS companies directly. We encourage you to talk with your provider first to see which ACCESS programs Duke offers and other Duke Health options we have available. We want your care to be seamless and integrated.

How to Get Started

Ask if ACCESS is right for you. Your provider submits a simple referral—no separate appointment needed.

Enrollment is voluntary and completely free for eligible Original Medicare patients. Joining ACCESS does not affect your existing Medicare coverage or your relationship with your Duke Health care team.

The ACCESS program is new and will continue to evolve at Duke Health, with new programs launching over the next year. Stay tuned.

Participation does not affect your existing Medicare benefits or coverage. Home monitoring and care management services are provided by Duke Health's ACCESS program partners.
