Emerging Healthier, Together for Cardiometabolic Diseases - Social Support System - Clinical Trial

What is the Purpose of this Study?

People in the study will get meals delivered to their homes from local groups. They’ll also agree to share what the experience was like for them.

What is the Condition Being Studied?

Impact of meal service for patients with food insecurity discharged to home.

Who Can Participate in the Study?

This study is for people who are 60 or older, live in North Carolina, and were recently sent home from the hospital. They can join if they have one or more of these challenges:

- Need help leaving the house

- Can't cook or shop for themselves

- Live alone or with someone who helps them

- Have trouble affording food or money problems, based on answers in their medical records

What is Involved?

This study wants to see how getting meals from programs like Meals on Wheels helps older people with heart and diabetes problems after they leave the hospital. The researchers will talk to patients to learn what they think about the food, how it's delivered, and if having support makes them feel better.
