What do these scores mean?
The percentage of short-term home health episodes of care (out of 100) during which patients had symptoms of heart failure and appropriate actions were taken.
What are we doing to continue to improve our quality?
Each patient's care plan includes assessment orders from the attending physician, such as daily weighing and medication management, which are reviewed during every home health visit.
A patient's attending physician is promptly notified when there is a change in a patient's condition. If needed, new orders are obtained from the physician and education on that treatment is provided to the patient and their caregiver.
