You will be provided a swab that you will insert no more than 3/4 inch inside each nostril and rotate several times around. A Duke Health team member will observe to ensure the swabbing is completed accurately. If you are unable to swab yourself, please bring someone who can do it for you. Children under age 3 will have their swabs performed by a nurse.
Schedule a Self-Swab COVID-19 Test
Answer the questions below to schedule a self-swab nasal PCR COVID-19 test appointment.
Appointments take place at the Duke Covid Testing and Vaccine Center, located at 1400 Morreene Road, Durham, NC. The self-swab test is provided at no cost to you, however, your insurance may be billed.
If you had close contact with a person confirmed to have COVID-19 or with symptoms within the last 14 days, schedule your testing appointment at least 5 days after you were exposed.
Before Scheduling
You must have a Duke MyChart account to view your results, which will be available in one to two days. If you have been seen at Duke before, and don't have a Duke MyChart account, use this link to create an account. If you are new to Duke, you can create a MyChart account after you sign up for your COVID-19 testing appointment.
If You Require Testing for Travel
Check with your destination to determine the appropriate timeframe for testing. Duke Health cannot guarantee or expedite results in time for your travel. If you need a specific type of test or need results sooner, contact your primary care provider or another North Carolina testing site.
If You Are a Duke Employee
If you have symptoms or were exposed to COVID-19, do not use this form to schedule. You must call Employee Health at 919-385-0429, option 1. If you are an employee with no symptoms or known exposure, you can schedule a test.
