Sports physicals for tween/teen athletes

Does your child need a sports physical? If he or she plans to work out with the middle or high school team this summer, participate in a summer sport, or try out for a sport any time in the upcoming school year, now is the time to make sure your forms are up to date.

School sports physicals for $10.00 (cash only)

The Duke Sports Science Institute will offer school sports physicals for tweens and teens in middle and high school students at our clinic on the Center for Living Campus in Durham. Optional concussion baseline testing will also be offered.

Free parking is available next to our building.

Sports physicals are offered on Tuesday, May 17, 2016 from 6:00pm - 8:00pm for $10.00 (cash only).

May 17 is the only day sports physicals will be available for $10.00. Cash only.

Students athletes in sixth grade and up should arrive with a completed physical form signed by parent/guardian. You can get the form from your student’s high school athletic trainer or download it here.
