Before coming to your first appointment, please fill out the forms you will need for your appointment to ensure that you are seen in a timely manner by specialists who understand your problem in detail.

• If you are coming in for your first appointment, fill out the New Patient Intake Form (PDF, 64KB).
• If you have an appointment for a voice evaluation, please fill out the Voice Questionnaire (PDF, 55KB).
• If you are a voice patient, please take a few minutes and complete the Voice Handicap Index (PDF, 41KB).
• If you are a singer, also fill out the Singing Voice Handicap Index (PDF, 46KB).
• If you a having problems with coughing, fill out the Cough Questionnaire (PDF, 66KB).
• If you think you may have Vocal Cord Dysfunction, fill out the VCD Questionnaire (PDF, 55KB).
• For return appointment, please fill out the Return Patient Intake Form (PDF, 65KB).
