You should schedule a brief travel clinic visit with Duke Travel Clinic if:

  • You do not desire an in-depth (comprehensive) travel appointment with a travel specialist
  • You are a traveler who is between 16 to 60 years of age
  • You are in good health without preexisting medical conditions
  • You are traveling to only one or two countries

Description of Visit

During your visit, you should expect to receive:

  • A brief review of your medical history
  • A risk assessment
  • General and specific itinerary-based patient education
  • The administration of desired immunizations
  • Trip-specific prescriptions

Steps to Schedule Your Appointment

Please print and fill out an Appointment Request Form:

Adults -- over age 16 (PDF, 1.03 MB)

Children -- ages two to 16 (PDF, 1.02 MB)

Fax the completed form to Duke Urgent Care South at 919-361-0762.

Once we review your intake information, we will call within 24 to 48 hours to confirm that you meet the criteria for a brief travel visit and with your appointment date/time.

If you do not meet the criteria for a brief visit, we may recommend a comprehensive travel clinic visit.

It is in your best interest to have an appointment six weeks prior to departure. However, we will attempt to accommodate short-notice requests for emergency travel.

Appointments for brief travel visits are available seven days a week, 365 days a year.

Hours of Operation

Monday to Friday, 8:00 a.m. to 4:00 p.m.
We are no longer offering weekend appointments for the brief travel visits.

Severe Weather

If you have a question about the Travel Clinic being open during severe weather, please call 919-572-1868 for recorded information on the clinic’s operational status.

Insurance

Please note that Duke Travel Clinic services are often not reimbursed under insurance plans. You must be prepared to pay for services at the time they are rendered by cash, check, or credit card.

A copy of the charges will be provided at checkout. A statement will also be mailed for those that wish to pursue reimbursement from their carrier or employer.

Duke employees traveling on business should bring a department procurement card.

Consultation Preparation

If available, please bring:

  • Your current immunization card (previously completed immunizations with dates)
  • International certificate of vaccine (yellow card)
  • Medical information (diagnoses, allergies, and medications)

Vaccine Information

Vaccine information statements are provided by the Centers for Disease Control (CDC) for patients who are considering being vaccinated to protect against one or more diseases.

You are able to review the selected statements prior to being vaccinated and direct questions to the provider before being immunized. You may wish to review the statements in advance at the CDC Web site.
