We take pride at Duke in our ability to evaluate new patients for consideration of small bowel transplantation as quickly as the referring physician deems appropriate.

Patients may be scheduled in clinic within one week if clinically indicated. Stable patients for routine referral are usually seen within two to three weeks at the transplant center.

To refer a patient for small bowel transplant, the referring physician needs to submit the following information via fax to 919-681-7930.

• Patient name, address, and phone number
• Patient height and weight
• Date of birth
• Recent history and physical and/or discharge summary
• Insurance information (if available)
• Referring physician's name, address, telephone number and fax number
