To become a patient in the Duke Lung Transplant Program, you must be referred by your primary doctor. We accept referrals from across the U.S.
Doctors wishing to refer a patient to Duke for lung transplantation should first review our referral guidelines and contraindications (PDF, 66 KB) to determine if the patient is eligible.
If your patient meets the eligibility criteria, complete the following referral form and fax it to 919-681-5770.

Once all the information is received, a member of our team will contact the patient and schedule the patient's evaluation. This process generally takes two to four weeks.
Once the evaluation has been completed, the referring physician will receive a letter with the transplant team's decision regarding whether transplantation is appropriate for the patient and a copy of all test results obtained during the patient's evaluation.
