Whether you have a health problem that requires treatment or you are perfectly healthy, the results of these tests will be filed in your patient record. Duke stores as much of your record as possible electronically, with full safeguards in place to protect your privacy.

Keeping patient records helps your internal medicine physicians monitor your health long-term and make the most accurate diagnoses possible for any future health problems.

If you would like a copy of your medical record, you will need to sign a medical records release form and fax or mail the form to your clinic.

Please allow seven to 14 days for paper copies to be made available for you to pick up at the clinic or sent to you by mail.
