To make an appointment with the Duke Adult Diabetes Education Program (ADEP), a referral from your diabetes care provider is required.

Please arrive 30 minutes prior to your appointment time to allow for check-in.

What to Bring

Bring a list of your medications, your blood glucose meter, logbook, and food records with you to each visit.

Insurance Information

Medicare covers diabetes self-care management training. Most other third party payers and managed care providers do provide coverage for these services.

A referral from your diabetes care provider is required.

Check with your health plan prior to enrolling to determine if these services are covered by your plan or if precertification is required. You will be required to pay your co-pay at the time of service.

Canceling an Appointment

We ask that you call us 24 hours prior to the appointment if you must cancel or reschedule.

If you cancel or don't attend multiple appointments, we will ask that you see your provider again prior to rescheduling the appointment.

To schedule or cancel an appointment, contact 919-620-5300. The completed referral form can be faxed to the Endocrine Appointment Hub at 919-479-2661 or to the ADEP office at 919-668-1650.
