To request medical records from a Duke health care provider, you will need to submit a written request to that provider's Information Release Unit.
You can either use an Authorization to Protected Health Information form or submit a letter to the unit. (See what to include in the letter.)
If the information to be released is sent to a physician, hospital or medical provider, police department, or school, there will be no charge for the records. For records sent anywhere else (to yourself, Social Security, attorney, etc.), there will be a fee assessed based on the number of pages copied. Payment must be remitted before records are sent.
Record requests are usually processed within seven to 10 business days.
Patients requesting medical records from Duke University Hospital, Duke Children's Hospital and Health Center, or Duke Clinic must submit a written request to the Medical Information Release Unit (MIRU) of Duke University Hospital.
Individuals have the right to request that entities within Duke University Health System amend protected health information contained within the designated record set for as long as the DUHS entity maintains the protected health information.

Patients requesting medical records from Durham Regional Hospital need to submit a written authorization directly to the hospital's Information Release Unit. Please bring identification with you to the hospital when you submit your authorization form.
If you are unable to submit authorization in person, you may fill out an authorization form and mail it to the hospital.
