Why You Should Have an Advance Directive
An advance directive is a legal document that shares your wishes for medical care if you can't make decisions for yourself. In North Carolina, there are two types of advance directives:
- A health care power of attorney lets you choose someone you trust to make medical choices on your behalf if you are unable to.
- A living will tells others what kind of medical care you want if you are very sick and can’t speak for yourself.
This advance directive form covers both. Download the form and get it notarized. Your Duke Health team can help you with this process and answer any questions you have.
Once completed, please give the form to a member of your care team. We must have a copy to follow your wishes. If you cannot give us a copy, we can help you complete another form so one is on file if needed while you are in a Duke Health hospital.
