To my patients,
First of all, I would like to thank you for the trust you have given to me as your primary care physician over the past seven and a half years working with Duke General Internal Medicine. Taking care of you has been an honor for me and my colleagues, and maintaining your wellness and health have been very important to us.
Effective April 30, 2008, I will no longer be practicing with Duke General Internal Medicine and therefore will not be able to continue in my role as your primary care physician.
Duke General Internal Medicine respects that the choice of a health care provider is a patient’s personal decision, and we request that you make every effort to choose a new primary care provider before July 30, 2008. The providers at Duke General Internal Medicine will make every effort to meet your acute care needs after my departure until July 30, 2008.
Other local practices and physicians in the Duke University Health System are accepting new patients, and for further information you may call 1-888-ASK-DUKE. You can also visit Duke Medicine on the Web at DukeHealth.org to find profiles of other Duke Medicine practices and physicians. To obtain your current medical records or have them transferred to your new physician and practice, please call 919-490-9800.
