Patients who are currently in the medical home of Duke General Internal Medicine at the Duke Health Center at Pickett Road can be referred by their physician to Medication Management Services.
Dr. Philip Rodgers, our clinical pharmacist practitioner (CPP), works in collaboration with your primary care physician as part of your medical home to optimize your medication management for diabetes, hypertension, and heart disease. In addition to being a CPP, Dr. Rodgers is a clinical associate professor at the UNC School of Pharmacy. He is involved with the training and education of pharmacy students and pharmacy residents at the Duke University Medical Center. Patients who are involved with the Medication Management Services have comprehensive, individual, one-on-onr appointments with Dr. Rodgers and his team that last 30 to 60 minutes.
Services provided by Dr. Rodgers and his team include diabetes education, which includes reviewing the complications and treatments for diabetes, evaluating your sugar control by reviewing your home glucose monitoring reports, and performing in-clinic assessments and laboratory measurements as necessary.
Dr. Rodgers, as a clinical pharmacist practitioner, can adjust your medications or recommend other medications that may better help you reach your clinical targets for diabetes, blood pressure and cholesterol. Dr. Rodgers and his team remain in close contact with your primary care physician at the Duke Health Center at Pickett Road to further enhance your medication management, and to keep you well and informed about your medical condition.
