To my patients,
First of all, I would like to thank you for the trust you have given to me as your primary care physician over the past 15 years. Taking care of you has been an honor for me and my colleagues, and maintaining your wellness and health have been very important to us.
As many of you know, I completed a fellowship in integrative medicine at the University of Arizona in December 2006 and have recently been working part-time at Duke Integrative Medicine at their new facility on the campus of Duke Center for Living.
On October 1, 2007, I will make a transition from my practice of general internal medicine to a full-time practice of integrative medicine.
Therefore, I will not be able to continue in my role as your primary care physician when I make the transition to Duke Integrative Medicine, where I will not have a general medicine practice.
In my new practice, I will be acting as a consultant with patients and their primary care doctors to offer a new holistic approach to help patients achieve optimal health. More information about Duke Integrative Medicine can be found at their Web site.
The providers at Duke General Internal Medicine will make every effort to meet your acute care needs during the time of transition and are actively recruiting new providers to join the practice.
Duke General Internal Medicine respects that the choice of a health care provider is a patient's personal decision. We do request that you make every effort to choose a new primary care provider before December 31.
For help in the process, call 1-888-ASK-DUKE to find a Duke primary care provider accepting new patients at a practice near you.To obtain your current medical records or have them transferred to your new physician and practice, call 919-490-9800.
Thank you again for allowing me to serve as your personal physician.
