Forms and Documentation

As a courtesy to children and students, we will complete required forms for school and camp if we have provided a physical exam within 12 months.

Please allow seven days for completion. Call 919-681-2390 for more information.

Requests for disability will be referred to a specialist.

Medical Records

For Duke Family Medicine Center medical records, please call 919-681-2390.

Copies of medical records transferred to and from our center require a signed release from the patient.

There is a charge for copies released to the patient.
