How do I retrieve my medical records from my previous physician?

You need to contact your primary care physician and bring a copy of your last doctor's visit to your appointment.

How do I obtain my medical records from my Duke physician if I want a second opinion or would like to see them?

Due to HIPAA regulations, we are not permitted to disclose medical information to patients. Therefore, you will need to contact Medical Records (Information Release) at 919-684-3684.

Can my physician order several tests while I am at my visit so I do not have to come back for an additional visit?

Your physician will determine what tests and how many need to be performed after he or she assesses your condition. Therefore, only your physician can determine what tests you will need and if you need to come back for a subsequent visit.
