Description: We are pleased to announce the Duke Signature Care is moving and expanding. This transition includes the creation of a new primary care model designed for people seeking round -the -clock access to their physician and a greater level of individualized service.

Duke Signature Care will be an innovative practice that will allow you to better engage and participate in your health care. Plans include health and education classes with potential telemedicine options for care.

An annual membership fee will provide Duke Signature Care patients with 24/7 access to a world-class Duke Signature Care primary care physician and care team.

We will be hosting informational sessions about this new practice to answer any questions you might have and to provide more information.
