A physician referral is not necessary to make an appointment with the Duke Breast Oncology Program.

To make an appointment, please call the Duke Breast Oncology Program directly at 919-660-9672 or Duke oncology at 919-668-6688 (Durham) or 919-954-3050 (Raleigh).

Our trained representatives can help you set up an appointment with Duke Cancer Institute specialists.

At your initial visit the multidisciplinary team will review your medical information, including your pathology reports.

To assist in developing your plan of care, please complete the breast program questionnaire -- English version (PDF, 390 KB) or Spanish version (PDF, 414 KB) -- and the Duke medical release form (PDF, 241 KB) so we can obtain your medical information. Please fax both forms to 919-681-9872 prior to your appointment.

Once you have made your appointment you will receive information in the mail about the location and time, as well as how to prepare for your visit to the Duke Cancer Institute.

Patients can also register in advance, print maps, and more by visiting Duke Medicine's secure site, healthview.dukehealth.org.
