What Does This Measure?

The overall percentage of eligible heart failure patients who received all of the measures of high-quality care tracked by the Centers for Medicare and Medicaid Services (CMS).

Why Is This Important?

Certain treatments have been proven to get the best results for most heart failure patients. This measurement shows a hospital’s overall performance in providing recommended treatments for heart failure.

What Are We Doing to Improve Our Performance?

Duke has enacted several initiatives to improve our performance. These include:

  • Electronic medical records (EMR)
  • Evidence-based medicine

  • Heart failure-specific discharge instructions and patient education information

  • Patient advocacy and support groups
  • Patient rounding
  • Patient safety and clinical quality committees
  • Six Sigma
  • Smoking cessation programs and patient education information
  • Standardized order sets
