To request an appointment, contact the appointment line at 919-572-HOPE (4673) and select option 1.
You will need to provide the clerk the following information:
- A brief summary of your problems
- If you have a preference, the name of the provider you would like to see. (See a list of providers)
Please arrive a few minutes early for your first appointment as you will be asked to fill out a patient history intake form at your initial consultation.
Your Initial Visit
Patients enter our clinic through referrals from their primary care providers or their OB/GYN physicians as well as contacting us directly.
The usual scenario for a new patient entering our clinic will involve a meeting with one of the physicians.
The initial visit will involve reviewing any previous treatments you may have had as well as formulating a plan for your management here at the Duke Fertility Clinic. Please try to obtain copies of your records pertaining to previous evaluations and treatments.
Operative reports can usually be obtained by contacting medical records at the hospital where the procedure was performed or the operating physician's office. Copies of films such as hysterosalpingogram will also be helpful.
During this initial visit the physician seeing you will compile the details of your history and past treatments, if any, and decide on the necessary testing. Expectations and treatments may be discussed but should not be considered finalized until all the recommended evaluation is complete.
Your physician will continue to manage your care and evaluate the results of the tests that have been ordered. Once all the necessary testing has been completed your physician will be able to formulate a plan for your treatment that is suitable to your individual findings and desires.
Be sure your questions are answered about any part of your evaluation and treatment before actually starting treatment.
You will need to have a current Pap smear on file. Please obtain a copy of the results if you have had one within the last year. Often your primary care physician or referring gynecologist will be able to assist you in getting a current Pap smear.

Insurance
Your insurance carrier may require a referral from your primary care provider.
Please contact your insurance carrier about the type of coverage provided and what you will need to do to ensure you have access to your benefits. You do not want to become burdened with unexpected bills which could have been avoided by investigation of your insurance options.
Our financial representatives will be able to assist you with most of these matters but there is no substitute for good preparation.
