To better evaluate and meet your needs, we are now asking all return clients to complete a return application prior to your arrival.

The information will help us schedule exercise tolerance tests when needed or request additional medical information so we can get you cleared to exercise as quickly as possible.

Complete a return application and a member of the DFC Admissions team will review your application and contact you with confirmation.
