Community Report

Partners in Care is a report to the community that highlights the numerous ways Duke University Health System and our world-class network of regional health care organizations are partnering with Durham health, government, and community organizations to improve the health of our neighbors.

This first report was published in the winter of 2003 and was printed in both English and Spanish. It includes a look at numerous programs including community health clinics; obesity prevention; Just For Us, a program helping senior citizens and persons with disabilities in public and subsidized housing get medical care in their homes; and the Durham Community Health Network, which provides health care support for families and individuals, especially those with chronic and life threatening conditions, who are on Medicare.

The report also contains detailed information on how to contact community programs to access services, the different entities in the Health System, and on our community partners.
